Change of address
In order to change your address in the DOE database, please send a lease or a Con Edison bill to the Main Office (Room 107).
School Letter/Proof of Enrollment
If you need a school letter for proof of your child’s enrollment (for taxes, housing, social services or another reason), please do the following:
- Send an email to The Pupil Accounting Secretary, Ms. Blaylock: rblaylock@schools.nyc.gov
- ; or
- Your child can request these letters in the Main office by signing the School Letter Book.
MTA MetroCards
If a student’s MTA MetroCard is lost or damaged, they can stop by the Main Office, Room 107, to pick up a MetroCard replacement form. The form needs to be taken home to be signed by a parent. Return the completed form to the main office for processing. Students will receive their new MetroCard the following day. Please note, MetroCards are assigned based on the student’s home address in the DOE database. If you have any questions or concerns, email Ms. Blaylock at rblaylock@schools.nyc.gov
